Waiver and Release of Liability for Participation in the Inclusion Club
In agreeing to allow my child to participate as a host teacher in the Inclusion Club provided by Craftibility Foundation, I hereby agree as follows:
I, on behalf of my child, fully understand and acknowledge that:
a) The Inclusion Club classes focus on teaching empathy towards teens and adults with special needs through arts and crafts activities.
b) During Inclusion Club activities, your child may be exposed to the possibility of injury, loss, and damage based on your child’s participation. We have, however, taken precautions to reduce risk and provide a safe, educational, and inclusive environment. You acknowledge and understand these risks, and agree that you will not hold Craftibility or the participating host school responsible for any personal injury or property damage as a result of your child’s enrollment in and participation with the Inclusion Club.
c) If your child is selected as a host teacher and is based outside the state of Missouri, I will need to purchase designated arts and crafts materials related to the lesson plan provided by the Craftibility Foundation.
d) During the program, pictures and video of your child engaged in various Inclusion Club activities may be taken, as well as the artwork created by your child. You agree that we may use these pictures and video with or without modification in relation to this Inclusion Club and Craftibility, including without limitation promotion of Craftibility in a wide variety of mediums, without further compensation for such usage. The pictures and video may be made available to you upon request. The pictures and video will not otherwise be used without your permission, and they will never be sold.
I, on behalf of myself, hereby voluntarily agree to release, waive, discharge, hold harmless, defend and indemnify Craftibility Foundation and the host school and all its members from any or all claims, actions or losses for bodily injury, property damage, loss of services or otherwise which may arise out of participation in the Inclusion Club activities. I specifically understand that I am releasing, discharging and waiving any claims or actions that I may have presently or in the future.
I HAVE READ THE ABOVE WAIVER AND RELEASE AND BY SIGNING IT AGREE. IT IS MY INTENTION TO RELIEVE THE CRAFTIBILITY FOUNDATION AND HOST SCHOOL AND ITS MEMBERS FROM LIABILITY FOR PERSONAL INJURY, DEATH, AND PROPERTY DAMAGE.
